Heparin calcium prevent clotting of blood in vitro and in vivo through its action on antithrombin 3, also inhibit the formation of a stable fibrin clot and has antilipaemic effect Antidote : Protamine sulphate 50 mg/10 min.I.V.
Side effects: may can give rise to hemorrhage due to its action. It also cause thrombocytopenia, producing a plateletaggregating antibody. So may exacerbate the condition being treated.
Interactions: used with care with oral anticoagulants or drugs,e.g aspirin& dipyridamole, that affect platelet function. NSAIDs may also increase the risk of haemorrhage..
Precautions: should not be given to patients at serious risk of haemorrhage e.g. peptic ulcer disease, cerebrovascular disorders, bacterial endocarditis„ oesophageal varices,...
Dose starting 1250 i.u. I.V. then 5000:10,0000 i.u. / 4 hours- or I.V. Infusion 20,000 : 40,000 i.u. daily given S.C.
5000 i.u. before surgeries to avoid clotting -then every 8:12 hrs- Kids 50 i.u./kg. I.V. can be increased to 100 i.u./kg. Every 4 hrs.
