Mechanism:inhibits replication and function of T lymphocytes and possibly B lymphocytes as well. In addition, methotrexate can inhibit interleukin-1 activity, impair the release of histamine from basophils, and decrease chemotaxis of neutrophils. All three physiologic properties are believed to contribute to the immunologic actions or immunologic actions
Uses: 1- For the treatment of acute lymphocytic leukemia 2- For the treatment of non-Hodgkin's lymphoma and Burkitt's lymphoma 3- For the treatment of epidemoid or squamous cell carcinoma of the head and neck, and for refractory lymphomas 4- For the treatment of bladder carcinoma 5- For the treatment of breast carcinoma .. .etc Side effects: Bone-marrow depression can occur abruptly, and leucopenia, thrombocytopenia, and anaemia may all occur. The nadir of the platelet and white-blood cell counts is usually around 5 to 10 days after a bolus Dose , with recovery between about 14 to 28 days. Ulceration of the mouth and GIT disturbances are also early signs of toxicity:
stomatitis and diarrhoea during treatment indicate that it may need to be interrupted, otherwise haemorrhagic enteritis, intestinal perforation, and death may follow. Methotrexate is associated with liver damage, both acute and, more seriously, chronic (generally after Iong-term use).
